Carer Needs Assessment Officer (2 posts)
Each of the two posts are for 26.5 hours a week and
for a fixed period of 12 months.
Salary : £ 20,099 pro-rata plus 6% pension allowance (salary subject to pay award)
Two exciting opportunities exist for enthusiastic persons, with significant experience of working with Carers, to provide the Denbighshire Carers Needs Assessment Service together with an Emergency Planning Scheme.
One post holder will be based within the NEWCIS offices, Marsh Road Rhyl with the other postholder based in the South of the County (location to be confirmed) and they will be expected to work closely with local agencies in the statutory and voluntary sectors in supporting the needs of Carers. The postholders will be expected to undertake holistic assessments of need and produce a Carers support plan that considers the services available to meet assessed needs. This is a pilot project with an initial duration of 12 months during which time the service can be fully evaluated to determine the best way forward for meeting the ongoing needs of Carers.
